Referral Information

At Southern Crescent Nephrology, we strive to make patient care a priority by working closely with our referring medical providers. Our office staff will work with referring providers to obtain all information to make a new consultation productive and informative for the patient.

Please email referrals to referral@scnkidney.com or fax to

For new referrals, please provide the following:
1. reason for referral
2. demographic sheet (with insurance and contact information)
3. most recent progress note
4. most recent lab work
5. most recent imaging (if applicable)

Once received, please allow at least 3-5 business days for scheduling.
